Consultation of a physiotherapist consists of several stages:

  • At first, history of the patient's health and injuries is clarified – anamnesis is collected.
  • Later, special tests are performed to assess the cause of the patient's problem and allow to select effective treatment.
  • Afterwards, the test results are discussed, and the problems are explained in detail.
  • And finally, based on the test results, an individual treatment program or an individual exercise plan is created.

Treatment of scoliosis by the Schroth method

Idiopathic scoliosis is a curvature of the spine in three anatomical planes due to an unknown cause.

Schroth method is one of the most effective and scientifically based methods used for the treatment of scoliosis. This method applies asymmetric exercises to the concave and convex sides of the spine along with corrective breathing.

Signs of scoliosis - when the child bends down, uneven muscles in the waist area becomes evident, when the child stands, the rotation and tilting of the pelvis is visible. Advanced scoliosis is characterized by a rib hump appearing in the chest area.

The Schroth method is suitable for both children and adults.

The curvature of the spine varies with age, it is usually observed in children aged 4-8 years. It initially progresses 1-2° per year and worsens especially during puberty (girls - 11-12 years, boys - 13-14 years). Progression can reach up to 10° per year. It is important to start treatment as early as possible before puberty, when treatment goals can be achieved most effectively.

Treatment of scoliosis in Lithuania has not yet reached an effective level. General practitioners rarely notice a curved spine in a child. In many cases, scoliosis is noticed too late, when the structural changes of the spine have already occurred and it is difficult to correct it with simple efforts. Untreated scoliosis often progresses and may require surgery.

Conventional physiotherapy is also often prescribed for scoliosis, but it is not appropriate in case of scoliosis. Conventional physical therapy only further improves the crooked curves of the spine. In case of scoliosis, each curve of the spine needs to be worked on separately.

In case of progressive idiopathic scoliosis, the thorax deforms over time and organs of the thorax and abdomen becomes displaced. Functioning of the internal organs, especially the lungs and the heart, becomes more difficult. Scoliosis usually results in abnormal breathing pattern, bad posture, muscle fascia changes, muscle fiber fatigue, muscle imbalance, changes/differences in muscle strength, endurance and flexibility, pain. The quality of a person's life is affected and the problem of the aesthetic image becomes apparent.

Therefore, it is important to notice scoliosis as soon as possible and take effective measures to stop it.

Dynamic assessment of movements, testing of muscle chains for the purpose of injury prevention and creation of an individual program – 70 €

Examination of functional human movements according to the complete Functional Movement system.

The examination consists of 4 parts:

  • FMS – assesses 7 basic functional human movements - stereotypes of the movements and their quality, control and integrity.
  • SFMA – This assessment is performed if movement dysfunction or pain is found during testing of the FMS part. The exact cause of movement dysfunction is determined: mobility, stability or movement control.
  • Y-BALANCE – assesses the dynamic stability of the upper and lower limbs and the symmetry of this stability – objectively determines the risk of injuries.

The tensiomyography method is patented and innovative method, based on more than 160 scientific studies. It is a functional muscle diagnostic system, which can be used to assess the type of muscle fibers, properties, tone, fatigue, spasticity, symmetry/asymmetry, minor muscle damage and atrophy of the muscle.

Manual therapy

Maitland mobilization

Objective tests are used to decide which manual therapy technique to apply, as well as evaluate its effectiveness and adjust the treatment plan if necessary. Maitland's concept can be applied to both limb and spine joints.

During mobilization, the general movement of the joint is restored, the sliding of the joint surfaces relative to each other is normalized, and normal, painless movement is restored.

In this technique, the speed of the movement and where it will be performed in the range of motion is important. Depending on whether you want to eliminate pain and irritation or increase the range of motion by stretching the joint capsule and surrounding tissues, the specialist selects the appropriate movement rhythm.

Mulligan joint mobilization

Mulligan joint mobilization is a part of manual therapy aimed at restoring pain-free, normal movement stereotype and full range of motion. Appropriate mobilization forces relieve patient's pain, reduce limitation or functional loss.

Mulligan Concept technique involves joint mobilization or gliding. When one or more similar symptoms are identified, which may include loss of joint motion, pain associated with movement, or with specific functional activities, the physical therapist applies passive mobilization of the joint, which is maintained throughout active movement. Then, if the joint is pain-free, the patient actively moves it through previously restricted and/or painful range of motion while the therapist continues to maintain adequate mobilization. After this technique, the pain decreases and the range of motion increases.

Stecco manual fascia therapy

STECCO manual fascia therapy is a special manual therapy method that aims to evaluate the effect of deep fascia on the occurrence of the musculoskeletal system diseases. In this method, the relation between fascia and muscle is imagined as a three-dimensional system in which there is a continuous interdependence.

The role of the fascia: according to Stecco's theory, the body is made up of 14 segments: head, neck, thorax, waist, pelvis, skull, humerus, elbow, wrist, fingers, hip, knee, ankle and foot. Each body segment is served by six myofascial units (mf units), consisting of monoarticular and biarticular unidirectional muscle fibers, their deep fascia, and an articulation that moves in one direction in one plane. Because fascia is made up of several layers and is richly innervated, its state of tension is maintained by the tone of the muscles woven into it. 
This tension allows receptors in nerve endings and fascial tissue to respond to all body movements and regulate tissue tension. When damaged, the fascia no longer slides, does not stretch properly, and fibrosis localizes at intersecting points of tension, called coordination centers and fusion centers.
In this way, the dysfunction of the musculoskeletal system develops 
– the sliding and movement of the organs is restricted, which causes pain in various parts of the body.

Ultrasound is a mechanical wave, which energy is transmitted to biological tissues through the vibration of molecules.

Ultrasound is classified as a deep heating physical modality and is used to increase tissue temperature. The biggest advantage of ultrasound compared to conduction-based physical modalities is greater penetration depth.

Ultrasound therapy 

The most commonly used ultrasound frequency in therapy is between 1 MHz and 3 MHz. Physiological and therapeutic effects of US are determined by endogenous heat and non-thermal component. The mechanical wave vibrates the tissues and accumulates heat in them, thus causing the physiological changes that are typical for massage. A 1°C increase in tissue temperature promotes metabolism and healing. An increase of 2-3°C reduces pain and muscle spasm, while an increase of 4°C increases collagen elasticity and reduces joint stiffness.

    Electrotherapy

    (TENS, RES) – 12 €

    Electrotherapy applies the exposure of an electric current to the joints, muscles and other soft tissues through the skin. It is one of the most effective methods of physical modalities to relieve acute and chronic pain (TENS) and perform functional neuromuscular stimulation (FES).

    Functional electrostimulation (FES) is a method which applies pulsed electric current in order to restore the normal functional state of tissues. Stimulated tissues become active and muscles contract. Stimulating the neuromuscular connection promotes the healing of the damaged nerve, activates metabolic processes in the muscles, blood flow intensifies, tissue nutrition improves, and muscles, which were weakened by illness or injury, become stronger.

    Muscle electrostimulation (FES) is applied in order to increase muscle strength and muscular endurance, promote metabolism, relieve pain, etc. It is used to treat connective tissue, musculoskeletal system and neurological diseases, etc.

    Pulsed electric current therapy (TENS) – transcutaneous electrical nerve stimulation. It is a treatment with short pulses of monopolar or bipolar pulsed electric current. During treatment, a low-frequency pulsed electric current is used. This procedure reduces pain, suppresses inflammation, improves blood circulation, and reduces muscle spasms.

    Sports massage is massage for athletes, which helps to prevent injuries and increase the athlete's performance. This massage is performed taking into account the needs of each athlete and according to the muscle groups which are employed the most in his/her sport. Sports massage is not a relaxing, calm massage – it stimulates, tones and increases capacity.

    Sports massage is divided into 7 types:

    Therapeutic back massage

    Therapeutic massage is the type of massage, which purpose is to treat various diseases, restore and normalize damaged functions of the body. This massage restores the function of the impaired body part, i.e. removes pain and muscle fatigue, tension, improves blood circulation and metabolism, relieves spasms, restores joint mobility.

    Therapeutic back massage is intended for those who feel tension in the back and shoulders, who work a sedentary job, as well as those who exercise intensively or those after injuries or surgeries.

    It is a massage that not only relaxes and improves general well-being, but also:

    • improves blood circulation;
    • reduces tension;
    • relaxes muscles;
    • activates lymph circulation;
    • reduces fatigue;
    • improves sleep quality.

    Lymphatic drainage massage to treat lymphedema

    Lymphedema is a disorder of lymphatic circulation, which causes the stasis of lymph in the damaged tissues. It is a chronic, life-long disease that causes inflammation, structural changes in the subcutaneous tissue, and degenerative changes of the skin. Due to the increased amount of lymph, lymphedema manifests itself by swelling of the face, neck, arms, trunk or legs.
    There may be a feeling of tightness, restriction of movement, and an increased risk of anxiety and depression.

    Lymphatic drainage massage is very different from a regular massage. This massage is performed slowly and gently, with light, rhythmic stretching movements that stimulate lymphatic vessels and lymph, which is directed in the direction of other regions. During the massage, the lymphatic vessels are opened, and the flow of lymph and tissue fluid through them is also stimulated. 

    Excellent results are achieved by the massage: activity of the lymphatic system is normalized, toxins are removed from the body, limb swelling is reduced, and skin elasticity is restored.
    At the same time, pain is reduced and relaxation is promoted.

    It is useful if:

    • Fatigue and swelling of the legs is felt
    • You suffer from leg cramps and prolonged pain
    • A small network of veins or dilated veins are visible
    • Skin changes, such as scarring and thickening of the skin (fibrosis) are visible
    • The pain increases when pressing the calf muscles, pressing on the sole of the foot or bending the foot
    • The affected leg is of blue or purple color
    • The affected limb is hotter
    • A feeling of heaviness or tightness
    • Limited range of motion

    Massage with instrumental soft tissue mobilization

    IASTM (Instrument-assisted soft tissue mobilization) is a myofascial intervention used to treat soft tissues and which involves the use of specialized tools to manipulate the skin,
    myofascia, muscles and tendons using a variety of direct compression techniques.

    IASTM applies instruments that are typically made of stainless steel with beveled edges and contours that can adapt to different anatomical areas of the body and ensures a deeper penetration, which means greater direct impact on muscles, fascia, ligaments and tendons.

    IASTM is not performed alone and is therefore combined with massage to enhance tissue remodeling. During the massage, if necessary,
    a physiotherapist uses the IASTM tool instead of hands to achieve a stronger and better effect.

    By using different tools and pressure, the specialist can focus on different tissue during the treatment. The beveled surface of the tool can specifically pull, stretch and loosen
    the fascial tissues.

    Such a targeted treatment method has many advantages:

    • Faster rehabilitation and recovery after injury.

    Conditions, which can be treated with ISATM, includes:

    Massage with vacuum cup therapy

    During a cupping massage, cups are placed on the skin. Their suction effect has a positive effect for the skin, improves blood circulation and functionality in the area where the cups are placed, promotes healing and reduces muscle tension and tissue swelling.

    This procedure is especially effective for reducing muscle and joint pain and restoring athletes after intense physical load. In addition, this type of therapy helps formation of the new connective tissue and blood vessels in the tissue. Muscle contraction and tone, oxygen supply to cells increases. The elimination of toxins and metabolic waste is accelerated.

    During this procedure, lymph circulation is activated and microcirculation of the skin and subcutaneous tissue is stimulated. Due to better oxygen supply, the tissues become softer and the deeper layers of the skin are nourished. The skin becomes more elastic and smooth.
